Hello, my name is Jodie and I am one of the nurses at the Hospice of the Valleys.

Most of you know us but we wanted to put names to faces you may have heard.  We are the nursing team at the Hospice of the Valleys.

We have all chosen to specialise in Palliative care and between us have years of nursing and community experience and expertise.  Most of us come from District Nurse backgrounds or working within our local hospitals and out of hours services.

Blaenau Gwent is home to most of us and we are passionate about our service supporting the local community.

Our role is to assess your needs and to see how we can help you along your journey.  This may include assessing your unmanaged symptoms such as pain, mobility and breathlessness.  We then co-ordinate with other teams within the community setting and place referrals on your behalf.  This co-ordinated care is built around the needs of you and your family addressing what you tell us matters the most.

We also support local care homes in the area.  Nicola who was recently awarded International Palliative care nurse of the year provides education for the staff alongside delivering clinical care for the residents.

Some of you may have seen us at Ysbyty Aneurin Bevan (YAB).  Emma our specialist ANP ensures that our support and specialist knowledge is offered in a ward setting providing a seamless journey from the hospital setting back into the community.
